How to get an NPI number

NPIs are free and come from CMS through the National Plan and Provider Enumeration System (NPPES). There are three ways to apply.

1. Online through NPPES (fastest)

Create an account in CMS's Identity & Access Management System (I&A), then sign in to NPPES and complete the application. Individuals apply for a Type 1 NPI; an authorized official applies for an organization's Type 2 NPI. You'll need your name and Social Security number (individuals) or legal business name and EIN (organizations), your practice location and mailing address, and at least one taxonomy code with any state license number.

2. On paper

Download form CMS-10114 (the NPI Application/Update Form), fill it in and mail it to the NPI Enumerator at the address on the form. Paper applications take longer.

3. Through an organization (bulk enumeration)

Large employers such as hospitals can apply for many providers at once through CMS's Electronic File Interchange (EFI), with each provider's permission.

After you apply

CMS emails you the NPI when it's issued. It appears in the public NPI Registry, and on this site after our next update (we load the full NPPES file monthly). Keep your record current: HIPAA rules require providers to report changes to NPPES within 30 days. How to update it.
