Removal requests

NPI records are public information that federal law makes available to everyone. As a courtesy, a provider can ask us to remove their page from this site by emailing admin@npiregistrylookup.com from an address we can match to the record, or with the NPI and a way to confirm it's theirs. Removing it here doesn't remove it from the official NPI Registry or other sites.
